OK, so HTML 4 is a better than XHTML for now

After reading jgraham's and Fredrik's comments (especially this doc), I have changed my mind about using XHTML with a ``text/html`` character encoding. I will go with HTML 4.01 strict until such time that either Web Apps 1.0 becomes a standard or XHTML is actually usable in most browsers.

This also got me to declare the character encoding in the HTTP header and clean up my MIME type mappings for lightTPD. I left out the meta tag for the character encoding at the moment, but I might put it back in later (not too worried about people saving my HTML pages to disk, plus I don't know how much web browsers even follow the http-equiv meta tags).
